BPD, narcissism, etc. can be attributed to what is being theorized in the scientific community as a cyst in the brain that cuts off "circulation" and causes atrophy. It causes parts of the brain to die.
It's a degenerative genetic abnormality. It gets worse as the person gets older. Removal of the cyst may improve the prognosis, but it seems like that would have to be done pretty early in life...

In short, the only way to deal with a BPD/narcissist is to get as far away from them as possible. It's not even a viable approach for them to seek psychiatric therapy. There is nothing that can be done, especially the later in life they are.
