The tattoo place would really have to be in on the prank, they'd have to be friends for sure.
They could draw it on and make it look like it's in progress, they could dust his chest with rouge to fake redness...
In the one clip he did show of needling, the guy's hand was blocking the area so it very well could have been faked.
Like I said though, they'd have to be really close friends for a studio to be willing to fake this.
Or he may have paid them. Who knows.
