Anorexia is sometimes driven (subconsciously) by a desire to stay small like a child forever.
Before anorexia/bulimia takes its tragic and extreme toll on peoples' looks, a lot of times they do basically look like kids.

She may be reading too much into the kilograms vs. pounds thing. The metric system is an international standard, more readily understood by an international audience.
